A0 --> nothing
RD --> nothing
MREQ --> no pulse, LOW

Does this means that my Z80 is broken? I use a ZILOG Z84C0006PSC Z80B CPU

are pulsing. /CS is decoded using some logic, and it is possible that
something wrong with that logic.
