additionally I want remark that I have used a modified 1.44MB diskette.
 
After formatting with FDTST I have made a VERIFY -> DISK. And when errors occured, I have continued with I(gnore) .
 
Result is - I can definitely say - that head 0 is cleanly able to read the sectors.
 
The errors take place as soon as head 1 is used.
 
One question for understanding:
 
Why is a 720K disk written the same way as a 1.44MB disk?
 
I mean: both use 4F tracks, 09 sectors and 2 heads. Or is the write density different? Up to now I always thought that 720K only uses 1 head.

It is theoretically possible you have one head that is "bad".  The thing about the format command is that it will complete fine even if it has failed to correctly format the disk.  There is no "checking" in the format function.

Basically, I suggest you format the entire disk, then attempt to read sectors back being sure to try different sector numbers, track numbers, and head numbers.  I would like to know if ANY of the reads seem to work.

I would like to see the log of another session at this point if that is convenient.

Yesterday night I continued testing a bit. I noticed the following behavior:

when trying to format and then read a defined track/sector a read problem seems to appear when a certain head is used. In the moment I don't know which of the two it exactly was.
